In today's healthcare environment patients and their families are demanding more than the bare basics of home care which Medicare provides. At Park Cities Healthcare Consultants we believe anything that the patient wants or needs is what they deserve. From diagnostics in the home, such as x-rays and cardiac monitoring, to private chef and chauffeur for those who need additional support. We arrange for all needs that are requested or required by our patients.

The following is a list of core services provided, but the extent to which we will serve our clients is only limited by their wants, needs and desires:
  
    1. Preventative management of chronic healthcare issues which serves to prevent emergency room visits and 
        hospital admission for those problems which arise. We will coordinate your plan of care with your personal 
        physician/nurse practitioner, or provide a visiting physician to your home. Your RN will perform a complete 
        physical assessment during the weekly visits to your home. We then communicate any concerns immediately 
        to your care provider so that a treatment plan can be put into place. We will also provide weekly updates to
        the patients family and attending physician as requested.

    2. Medication management. We monitor and set up patients' medication weekly or daily as needed to optimize 
        their quality of life. Pharmacies that deliver will be incorporated as requested to keep medication mis-
        management from occurring. This is a major preventable cause of hospitalizations and emergency room visits.

    3. Nurse Liaison service to escort patients to and from medical appointments, which enhances the coordination 
        of care. Medical appointments can be confusing for patients and having an advocate there for them will make it 
        go much more smoothly. If our patient is hospitalized we will be there every step of the way to advocate for 
        their needs.

    4. 24 hour Nurse Line. This is a comfort to patients and their families to know that a nurse who knows them and 
        their situation/condition well is there to discuss any problems and provide support and guidance.

    5. Professional caregiving services provided by Certified Nurse Aids, Certified Medication Aids and Certified Home
        Health Aids. When 24 hour care is in place our RN case management is included in the cost of        
        service, making our patients healthcare seamless and lessening the need for doctors office visits and 
        hospitalizations.
